The cybersecurity incident may have compromised names, health insurance details, dates of birth, medical information and Social Security numbers belonging to approximately 130,583 individuals.Who can file a claim for a data breach payout?Class members are individuals whose personal information was accessed without authorization in the December 2024 Community Dental Care data breach, including those who received notice of the incident.How much are settlement payments?Class members have the following benefit options:Cash payment A - Documented losses: Class members can claim up to $5,000 for unreimbursed, documented out-of-pocket losses that occurred between Dec. 20, 2024 and Oct. 19, 2026 and were caused by the data breach.
